Care Coordination is a collaborative process of assessment, planning, facilitation, evaluation, and advocacy that focuses on how to maintain wellness and navigate the health system. Care coordination takes place through thoughtful planning and proactive assessment of client’s future. A Care Coordinator is a client advocate who helps you navigate the maze of information and options. All of Navamaze Care Coordinators are Registered Nurses with advanced degrees, licenses, and certifications. You can be assured you will be treated with courtesy, respect and dignity.

Transition Specialists
The Navamaze team helps older adults and their families make decisions regarding home care, downsizing and moving, and choosing a senior living community. We also provide care management services, ongoing support to families with regular check ins, whether at home or at a community. We partner with the most highly regarded professionals, providing additional services to the same demographic. As there are many transitions during life, we have additional teams whose expertise provide appropriate resources and support. This includes new moms, professionals transitioning to new careers, entrepreneurs who are starting or want to grow their business.

Certified Senior Advisor (CSA)
The Certified Senior Advisor is a certification for professionals serving seniors. To be a CSA, an individual must pass a rigorous exam and uphold the highest ethical standards for the benefit and protection of the health and welfare of seniors. Typically, CSAs have expertise in professional discipline, for example, home care, senior housing, law, real estate, healthcare, insurance services, financial planning, or are supporting businesses wishing to better serve the senior clientele. To maintain certification, CSAs must meet continuing education requirements, disclose any new legal or regulatory issues, reaffirm their pledge to uphold the CSA code of Professional Responsibility and comply with certification policies.
